Un questionario, due lati del tavolo: chi lo riceve, chi lo configura. Una macchina, due facciate. One form, two sides of the table: who receives it, who configures it. One machine, two facades.

Il dominio era già pesante: regolatorio, multilingua, multifuso. Da quella materia siamo partiti per dare al sistema una forma — e prima ancora che una forma, una grammatica condivisa fra le due facciate. The domain was already heavy: regulatory, multilingual, multi-timezone. From that material we set out to give the system a shape — and before a shape, a shared grammar between the two facades.

Tutto è iniziato da una domanda apparentemente semplice: come fare arrivare un questionario al paziente, raccoglierne le risposte e riportarle nel sistema senza perdere nulla lungo il percorso. Dietro questa semplicità si nascondeva una macchina molto più complessa di quanto sembrasse. It all started from an apparently simple question: how to get a form to the patient, collect their answers and bring them back into the system without losing anything along the way. Behind this simplicity lay a machine far more complex than it appeared.

Sotto la superficie di una singola compilazione c'è una macchina. Otto leve da configurare lato centro perché un solo questionario possa partire correttamente. Beneath the surface of a single compilation there's a machine. Eight levers to configure on the site side, just to send one form out correctly. Four needs to satisfy on the patient side, so it's worth filling in. And above everything, three constraints crossing every decision: the regulatory frame of the domain, the linguistic plurality of trials, the distributed geography of who fills and who manages.

Quando la ricerca arriva di seconda mano cambia la disciplina: non si inventano insight che il dato non sostiene, ma si deve riconoscere dove il dato è denso e dove ha buchi. When research arrives second-hand the discipline shifts: you don't invent insights the data doesn't support, but you must recognize where the data is dense and where it has gaps. Those gaps close differently — client conversation, domain analysis, reference to similar ePRO/eCRF products — and they're declared in the profiles as composite, not as interviewed person.

Sono nati dal modello del dominio e dal research report. Each answers a different question, but all answer the same one: what makes these two surfaces one system, rather than two products that happen to share a language.

For them the app must disappear: no friction, no surprises, no extra taps, no messages requiring interpretation. Few clear states, timely notifications, privacy always at hand, draft that saves when life interrupts.

Quattro stati, una grammatica Four states, one grammar

Un questionario, dalla parte del paziente, attraversa quattro stati canonici. Stessi nomi, stessi colori, stesse regole, sia per un questionario one-shot sia per una cadenza ricorrente o continua. Niente terminologia diversa per tipo di studio — quella complessità resta lato centro, non sale fino al paziente. A form, from the patient's side, moves through four canonical states. Same names, same colors, same rules, whether for a one-shot form or a recurring/continuous cadence. No different terminology per study type — that complexity stays on the site side, never reaching the patient.

Bozza multi-sessione Multi-session draft

Un questionario può richiedere venti minuti. Venti minuti, in clinica, sono un'eternità se il paziente è anziano, in trattamento, o semplicemente interrotto da una telefonata. Per questo ogni risposta è salvata nel momento in cui viene data: chiudere l'app, spegnere il telefono, riaprire dopo un'ora — non cambia nulla. Il paziente ritrova esattamente la stessa schermata, con le stesse risposte già date e il cursore sulla prima ancora da dare. Non c'è un pulsante Salva bozza: salvare è il comportamento di default, non un'azione esplicita richiesta. A form can take twenty minutes. Twenty minutes, in a clinical setting, is an eternity if the patient is elderly, in treatment, or simply interrupted by a phone call. That's why every answer is saved the moment it's given: closing the app, switching off the phone, reopening an hour later — nothing changes. The patient finds exactly the same screen, with the same answers already given and the cursor on the first one still to come. There's no Save draft button: saving is the default behavior, not an explicit action required.

Notifiche puntuali, non rumorose Timely notifications, not noisy

Due eventi soli giustificano una notifica al paziente: l'arrivo di un nuovo questionario e l'approssimarsi della scadenza di uno già presente. Niente reminder a metà strada, niente nudge motivazionali, niente messaggi promozionali. Il paziente di uno studio non è un utente da retentionare: è una persona che ha già accettato di partecipare. La frequenza del sistema deve rispettarlo. Only two events justify a notification to the patient: a new form arriving and an existing one approaching expiration. No mid-cycle reminders, no motivational nudges, no promotional messages. A study patient isn't a user to retain: they've already agreed to participate. The system's frequency must respect that.

L'orario della notifica è calibrato sul fuso del paziente, non del centro. Se il paziente è in un altro fuso rispetto a chi gestisce lo studio, la notifica arriva alle nove del mattino di chi compila, non alle nove del mattino di chi monitora. Timezone isn't a technical detail: it's respect for the life on the other side.

Privacy sempre raggiungibile Privacy always reachable

In a healthcare platform under the GDPR frame, the privacy notice isn't signed once and forgotten. It must be reachable at any time, from the first screen as from the last, and organized to be read — not to be skipped. What I collect, why, for how long, who accesses it, how to withdraw consent: each is a distinct entry, not one block of legalese.

Lato centro la piattaforma ha una grammatica diversa: non più una facciata da semplificare, ma un'architettura da esporre. Due ruoli, cinque moduli, una matrice di permessi dichiarativa. On the site side the platform has a different grammar: no longer a facade to simplify, but an architecture to expose. Two roles, five modules, a declarative permissions matrix.

Il principio sotto è modularità dichiarativa. Tutti i moduli sono presenti nell'interfaccia eCRF — configurazione studio, assegnazione paziente, monitor delle compilazioni, audit log, modalità test. Chi vede cosa dipende dal ruolo dichiarato sull'account, non da una configurazione settata di volta in volta. Aggiungere un mestiere al sistema significa dichiarare quali moduli attivare per quel ruolo, non costruire una nuova interfaccia. The principle underneath is declarative modularity. All modules are present in the eCRF interface — study configuration, patient assignment, submission monitoring, audit log, test mode. Who sees what depends on the role declared on the account, not on a case-by-case configuration. Adding a craft to the system means declaring which modules to activate for that role, not building a new interface.

La struttura è multi-livello: ogni studio vive in un tenant, ogni tenant raccoglie uno o più centri, ogni centro ha i propri ruoli. Il Coordinator può entrarci in consultazione — non sta a lui validare il contenuto, sta a lui sorvegliare il flusso. Test mode. Write-enabled for the Configurator to dry-run a setup before sending it to real patients: the form is filled exactly as the patient will see it, in all its languages, and validated before launch. The Coordinator can enter for inspection — it's not their job to validate content, it's their job to watch the flow.

The backbone of everything is the audit log, active for every role, always, no exceptions. Every configuration, every assignment, every access, every change leaves a trace. Under the GCP and 21 CFR Part 11 frame this isn't a nice-to-have: it's the condition for the collected data to serve a regulated clinical study at all.

Configurare un questionario non è impostare un form. È coordinare cosa, quando, a chi, in che lingua e in che fuso — senza che il meccanismo dietro arrivi mai in superficie. Configuring a form isn't setting up a form. It's coordinating what, when, to whom, in which language and timezone — without the machinery behind ever surfacing.

Sei passi, una grammatica Six steps, one grammar

La configurazione è un'unica operazione lunga, non sei flussi separati. L'ordine è prescrittivo perché ogni passo dichiara informazioni che il successivo richiede: senza lingua principale non si possono caricare traduzioni; senza cadenza non si può definire la finestra; senza finestra non si possono settare i destinatari delle notifiche. The order is prescriptive because each step declares information the next needs: no primary language, no translations; no cadence, no window; no window, no notification recipients. It's a guided path, with test mode as the mandatory last station.

Il caso vivo: unlimited rigenerativa The live case: regenerative unlimited

Tra le cinque famiglie di cadenza, una è strutturalmente diversa: la unlimited rigenerativa. Mentre le altre quattro sono temporizzate (ogni N tempo nasce un questionario nuovo), questa è event-driven: appena il paziente invia, ne nasce subito un altro, nella stessa finestra, fino alla chiusura del periodo. Il paziente, nell'arco di una giornata, può vedere il proprio questionario rinascere tante volte quante è disposto a compilarlo. Among the five cadence families, one is structurally different: regenerative unlimited. While the other four are time-bound (every N time a new form is born), this one is event-driven: as soon as the patient submits, another is born right away, in the same window, until the period closes. Over a day, the patient may see their form regenerate as many times as they're willing to fill it.

È il caso più tecnico — richiede un meccanismo di rate-limit per evitare cicli di compilazione patologici, una logica chiara di cosa fare quando la finestra si chiude in mezzo a una bozza, e una continuità di stato lato paziente (un questionario inviato si chiude come Inviato, e nella stessa finestra ne nasce uno Nuovo, senza inventare un sesto stato). It's the most technical case — it requires a rate-limit mechanism to avoid pathological compilation loops, clear logic for what happens if the window closes mid-draft, and a continuity of state on the patient side (a submitted form closes as Submitted, and in the same window a New one is born, without inventing a fifth state). It's also the case that best embodies the case study's thesis: the patient sees one form, behind it a system governs in real time.

Il fuso come materia, non dettaglio The timezone as material, not detail

In ogni passo della configurazione il fuso è esposto, non implicito. La finestra dello studio è dichiarata nel fuso del Configuratore; la durata di assegnazione per ogni paziente è calibrata sul fuso del paziente. Le notifiche partono alle nove del mattino di chi le riceve, non di chi le invia. Sembra ovvio. In molti sistemi non lo è: il fuso resta una convenzione del server, e per il paziente sull'altro lato dell'Europa la notifica arriva alle tre di notte. Qui no. In every step of the configuration the timezone is exposed, not implicit. The study window is declared in the Configurator's timezone; the per-patient assignment duration is calibrated to the patient's timezone. Notifications go out at nine in the morning of who receives them, not of who sends them. Sounds obvious. In many systems it isn't: the timezone stays a server convention, and the patient on the other side of Europe gets the notification at three a.m. Not here.

Six guided steps, a clear grammar, a dry-run as safety net. Density isn't hidden behind cosmetic simplifications: it's exposed, made readable, made verifiable. The patient, at the end of all this, sees a single gesture. Behind it, a machine that knows exactly what, when, to whom, in which language, in which timezone, at which cadence.

Nel mondo degli studi clinici regolati, i benchmark riconosciuti per i sistemi ePRO sono noti. Compliance rate — the share of forms filled within the window — typically sits above 80%, with well-designed systems exceeding 90%. It's a structural leap from paper diaries, where studies show only about 11% of entries are recorded at the correct time — the rest is back-filled on the visit day. Under the 21 CFR Part 11, GCP/ICH-E6, and GDPR frame, ePRO platforms are now the expected standard, not the exception.

Una nota su quello che non posso pubblicare A note on what I cannot publish

Le metriche numeriche dirette esistono nei dati interni della piattaforma — compliance rate sugli studi attivi, tempo medio di compilazione, drop-off su questionari lunghi, retention nelle cadenze multi-giornaliere — ma non sono pubblicabili sotto NDA. La piattaforma serve sponsor identificabili dai propri dati: pubblicare i numeri significherebbe pubblicare il cliente. Per questa scelta, e per coerenza con la cornice del case, le metriche restano interne. Gli outcome sopra sono fatti, non stime — verificabili da chiunque usi il sistema oggi. Direct numerical metrics exist in the platform's internal data — compliance rate on active studies, average compilation time, drop-off on long forms, retention on multi-daily cadences — but cannot be published under NDA. The platform serves sponsors identifiable from their own data: publishing the numbers would mean publishing the client. For this choice, and for consistency with the case's frame, metrics stay internal. The outcomes above are facts, not estimates — verifiable by anyone using the system today.
